Forkortelser og symboler der ikke må anvendes<br /> Følgende forkortelser og symboler må ikke benyttes i håndskrevne notater i patientjournalen:<br /> <br /> * μg…. fordi μg kan tolkes som mg.<br /> Skriv i stedet mikrogram eller mikrog.<br /> <br /> * ugl. og dgl.<br /> Skriv i stedet ugentligt og dagligt…. fordi forvekslinger kan medføre alvorlige medicineringsfejl. <br /> <br /> * &gt; og &lt;…. fordi &gt; kan forveksles med tallet 7, og fordi begge symboler kan opfattes modsat.<br /> Skriv i stedet større/mere end og mindre end.<br /> <br /> * Hele tal efterfulgt af komma og et 0 (nul), fx 2,0. Skriv i stedet 2 mg, hvilket kan fjerne risikoen for en ’faktor 10 fejl’ som følge af, at et komma let kan overses<br /> <br /> == Specialespecifikke forkortelser ==<br /> Herudover bør alle kliniske afdelinger og klinikker udarbejde en supplerende liste over forbudte forkortelser og symboler, hvis der i afdelingen/specialet anvendes forkortelser eller symboler, som har givet eller kan give anledning til misforståelser eller forvekslinger. Denne liste gøres tilgængelig for det kliniske personale og Lægesekretærer relevante steder.<br /> <br /> Såfremt der i afdelingen anvendes specialespecifikke forkortelser og symboler, skal det sikres at disse ikke er identiske med alment anerkendte medicinske forkortelser for andre begreber, fx temp. for temperatur eller temporalis.<br /> <br /> De kliniske afdelinger bør endvidere vurdere om anvendelsen af specialespecifikke forkortelser eller symboler i patientjournalen antager et omfang, der gør notaterne utilgængelige/uforståelige for sundhedspersoner udenfor specialet, hvis disse sundhedspersoner skal anvende notaterne som grundlag for observation eller behandling af patienten.<br /> <br /> <br /> == Ansvar og organisation ==<br /> Hospitalsdirektionen har ansvar for implementering af denne instruks.<br /> <br /> Center, afdelings- eller klinikledelser har ansvar for: <br /> * at der lokalt arbejdes med at reducere antallet af forkortelser, som ikke er i overensstemmelse med alment anerkendte medicinske forkortelser<br /> * at der ikke anvendes forkortelser indenfor specialet, som er identiske med generelt anerkendte medicinske forkortelser med en anden betydning. <br /> <br /> <br /> == Referencer ==<br /> <br /> § Helene Jantzen.
